Total and Permanent Disablement claims

Your TPD cover is your financial back-up plan and provides you with a one-off payment if you are totally and permanently disabled under the policy definition. To make a TPD claim, you will generally need to provide:

  • An initial claim form (which we will send to you) that includes sections for you, your treating doctor and your treating specialist to complete. The treating doctor’s statements provide details of your condition and your medical history.
  • A certified copy of either a Birth Certificate, current Australian Passport or Driver Licence.
  • Your original policy document and policy schedule.
  • Copies of medical reports and investigations performed.

Your Case Manager will advise you of what information we require to assess your claim and can help you through the process.
